Appointments with the Doctor

As from June 2022 all patients will be offered the choice of Face to Face or Telephone consultation appointments.

Appointments with the Practice Nurse

The Practice Nurse is available by appointment. The Practice Nurse will conduct New Patient Medicals, Cervical Smears, Contraception Advise, Adult and Child Immunisations. Clinics are available to monitor Asthma, Diabetes, Coronary Heart Disease, Hypertension and other Chronic Diseases. Wound dressing are NOT done at this surgery, please call Harold Wood Polyclinic on 0203 416 7711.

District Nurses

There is a dedicated District Nurse Team based at the Elm Park Clinic.

Cancellations

If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.

If you change your telephone number – Please let us know as soon as possible so we can update your records so that you will receive text messaging services and updates.

Home Visits

If you are too ill to come to the surgery, please ring before 10:30. Tell the receptionist the name, address and telephone number of the patient and any detail of symptoms that you can offer.

If you are a carer, you may need to be available to discuss the care or to let the doctor into the house, so please be precise about the problem. These visits are only for the seriously ill and genuinely housebound patients.

You can be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
